Charlottesville, VA – November 22, 2024 – Commonwealth Senior Living is proud to announce it has partnered with Serviam Care Network and Curana Health through the Value-Based Care Alliance of Virginia. Through the Alliance, Commonwealth’s residents will now have access to in-residence primary care services, regardless of their insurance plan, and the option to enroll in Curana’s Align Senior Care Institutional-Equivalent Special Needs Plan (IE-SNP): a Medicare Advantage plan designed to provide high-quality care for assisted living and memory care residents. In addition to full Original Medicare coverage, IE-SNP beneficiaries are eligible for expanded benefits, including a dental/denture allowance, a hearing/hearing aid allowance, a vision/eyewear allowance, podiatry services, an over-the-counter medication allowance, post-hospital in-residence services, and transportation to health care appointments.
“Our partnership with the Value-Based Care Alliance reflects our commitment to shaping the future of senior living by delivering exceptional, personalized care to our residents,” says Michelle Hamilton, Chief Operating Officer at Commonwealth Senior Living. “The Alliance makes it easy for us to fully leverage the benefits of value-based care, making a tangible difference in the lives of those we serve. This collaboration aligns with our Noble Selling Purpose, which is to improve the lives of our residents, their families, and each other.”
“We are thrilled to welcome Commonwealth Senior Living to the Value-Based Care Alliance of Virginia,” says Alan Fairbanks, President of Serviam’s Value-Based Care Alliance. “Our partnership aims to transform senior care, shifting the focus from reactive, episodic care to a proactive, holistic approach that improves residents’ quality of life and promotes long-term wellness.”
The value-based care model focuses on preventive care, disease management, and a coordinated healthcare approach. This not only enhances the well-being of residents, but also contributes to significant cost savings through reduced unnecessary hospital admissions and emergency room visits.
Commonwealth Senior Living is committed to improving resident safety and overall health outcomes and this partnership marks a significant step toward that goal.
